In SURMOUNT-5, tirzepatide produced greater average weight loss than semaglutide in adults with obesity without type 2 diabetes.
This was superiority for the studied weight outcome—not proof that one medicine is the best choice for every patient. Gastrointestinal side effects were common with both.
Maximum tolerated doses. The semaglutide doses were those used for obesity treatment; this was not a comparison with Ozempic 1 or 2 mg.
Read the original trialBrand and evidence matter. These results concern the medicines studied in the trial, not TYZEPA-specific clinical outcomes. Results vary between patients.
What about Ozempic and blood-sugar control?
Ozempic contains semaglutide. In SURPASS-2, 1,879 adults with type 2 diabetes taking metformin received tirzepatide 5, 10 or 15 mg, or semaglutide 1 mg, for 40 weeks.
Average HbA1c reductions were 2.01, 2.24 and 2.30 percentage points with tirzepatide, compared with 1.86 with semaglutide. This did not test Ozempic 2 mg or TYZEPA.
SURPASS-2 source ↗Side effects: what did the comparison show?
In SURMOUNT-5, treatment discontinuation because of adverse events was reported in 6.1% with tirzepatide and 8.0% with semaglutide. Some events were less frequent with tirzepatide, while others were similar or more frequent.

Adults with obesity, or overweight with a related condition, without diabetes; maximum tolerated tirzepatide 10/15 mg versus semaglutide 1.7/2.4 mg. These descriptive rates do not establish overall safety superiority. An adverse event is a reported medical occurrence and is not necessarily caused by treatment. Most gastrointestinal events were mild to moderate.
This trial did not test TYZEPA. It does not establish that TYZEPA has fewer side effects than other brands. Individual tolerability varies.

Does tirzepatide treat all six conditions?

Is tirzepatide always better than semaglutide?
No. The weight-loss trial favored tirzepatide for its measured outcome. The best individual choice also depends on medical history, other evidence, tolerability, local approved indications, access and cost.
What should I discuss before treatment?
These medicines have important risks and contraindications. Discuss a personal or family history of medullary thyroid carcinoma or MEN2, serious allergy, pancreatitis, severe stomach-emptying problems, gallbladder disease, other diabetes medicines and pregnancy plans.
Nausea, vomiting, diarrhea and constipation can occur. Tirzepatide can affect oral contraceptive absorption; the originator label recommends a non-oral contraceptive method, or an additional barrier method, for four weeks after starting and for four weeks after each dose increase. Follow your prescriber's advice and the supplied product information.

- Severe low blood sugar symptoms, especially when tirzepatide is used with insulin or a sulfonylurea.

The official Mounjaro® label warns about possible thyroid C-cell tumours: these occurred in rats, while the risk in people is unknown. It says Mounjaro® must not be used with a personal or family history of medullary thyroid carcinoma (MTC), MEN 2, or a previous serious allergy to tirzepatide or its ingredients. Report a new neck lump or swelling, persistent hoarseness, trouble swallowing or shortness of breath. Also discuss pancreas, gallbladder or kidney problems, severe stomach-emptying problems, diabetic eye disease, pregnancy plans, oral contraceptives and any planned surgery or anaesthesia. Confirm the contraindications in the TYZEPA leaflet before use.
